Job summary

New Destiny is seeking a full-time Case Manager to provide regular on-site services at Raven Hall in Coney. The Case Manager will work with the Program Director to maintain and grow a robust program focused on family stability by linking families to resources and promoting a violence-free environment.

Qualifications

• Bachelor’s degree required; BSW or BA/S preferred.
• 1–3 years of direct service experience preferred.
• Knowledge of housing stability, health & mental health, and food security.
• Experience with homelessness or domestic violence survivors is a plus.

Responsibilities

• Provide comprehensive case management to a caseload of 20–25 tenants, including counseling and crisis intervention.
• Develop programming with the Program Director and families; plan wellness activities.
• Coordinate with schools, clinics, and providers for referrals and follow-up.
• Maintain and update case files and internal databases; participate in weekly supervision.
• Engage volunteers and interns to enrich youth and family events.
