Job summary

New Destiny Housing Corporation is hiring a full-time Case Manager for on-site services at the Morris location in the Bronx. The role supports 20-25 tenants in a 39-unit affordable residence serving low-income families from DV shelters.

You will work with the Program Director and Senior Case Manager to foster family stability, connect families to resources, and help children thrive. Evening hours required two nights per week.

Qualifications

• Bachelor’s degree required; BSW or BA/BS in related fields preferred.
• Bilingual Spanish/English strongly preferred.
• Excellent written and verbal communication.
• Strong organizational and problem-solving abilities.

Responsibilities

• Provide case management to 20-25 tenants, including counseling and crisis intervention.
• Develop programming in collaboration with the Program Director and families.
• Create annual wellness plans with tenants.
• Conduct annual stability assessments.
• Plan age-appropriate therapeutic activities for children.
• Coordinate afterschool and weekend activities.
• Link tenants to community resources and services.
• Connect families with schools and clinics.
• Maintain program database and documentation.
• Attend supervision meetings and maintain case files.
