Job summary

Covenant House New York is seeking a bilingual Case Manager for the Family Covenant Scattered Site program, serving 10 young adult parents and their children in NYC. You will deliver comprehensive case management, coordinate services, and support housing stability through home visits and community-based work.

You will implement Parenting Journey 1, engage families with trauma-informed approaches, and collaborate with a multidisciplinary team to meet individualized goals and regulatory

Qualifications

Bachelor’s degree required.
Bilingual English/Spanish required.
1 year residential or scattered-site experience preferred.
Experience with resiliency-focused, trauma-informed care.
Knowledge of youth development and family services.

Responsibilities

Provide integrated case management and counseling to 10 young adult parents and children.
Promote housing stability and service-plan goals.
Develop individual service plans and coordinate referrals and services.
Deliver Parenting Journey 1 model during home visits.
Maintain confidential records and regulatory compliance.
Travel for home visits and work evenings as needed.
Collaborate with housing staff and service providers.
