Job summary

New Destiny Housing Corporation seeks a full-time Case Manager to provide regular on-site services at Raven Hall in Coney Island. You will work with the Program Director to sustain a robust program focused on family stability, connecting families to resources and supporting a violence-free environment for children to thrive.

The role requires one late night per week and reports to the Program Director. A BSW/BA in related fields and 1–3 years of direct service experience are preferred; bilingual

Qualifications

• BSW or BA/S in Psychology, Sociology, or Early Education preferred.
• Knowledge of social services and Brooklyn-area organizations is preferred.
• 1–3 years of direct service experience in housing stability, health and mental health, or food security preferred.
• Proven track record engaging homeless and DV survivors; empathetic, creative, resourceful.
• Trauma‑informed care experience preferred; highly organized, flexible, resourceful.
• Strong written and oral communication, computer literacy, and sound judgment.
• Bilingual Spanish/English strongly preferred.

Responsibilities

• Provide case management to a caseload of 20–25 tenants, including counseling, crisis intervention, and supportive services.
• Develop programming with the Program Director, Senior Case Manager, and families.
• Create and implement annual wellness plans with tenants.
• Conduct annual assessments to monitor stability and needs.
• Plan age-appropriate therapeutic activities for children.
• Organize therapeutic activities and workshops for families and children.
• Coordinate referrals to community services and stability programs.
• Maintain relationships with offsite partners and follow referrals to services.
• Follow New Destiny Housing’s Theory of Change across all activities.
• Coordinate with property management to ensure housing stability.
