Jobot is seeking a compassionate Case Manager to provide comprehensive support to individuals experiencing homelessness in Albuquerque. You will assess needs, develop service plans, connect clients with housing and community resources, and advocate toward self-sufficiency and permanent housing.

The ideal candidate has strong organizational and crisis intervention skills, experience with case management systems, and a valid driver’s license.

Competenze

• Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or related field.
• Experience in case management or nonprofit services.
• Knowledge of community resources and social service systems.
• Excellent communication, interpersonal, and crisis intervention skills.
• Ability to build rapport with diverse clients.
• Strong organizational skills with multiple cases.
• Experience with electronic case management systems and Microsoft Office.
• Valid driver's license and reliable transportation may be required.

Mansioni

• Conduct client intake assessments and develop individualized case management plans.
• Maintain ongoing contact with clients to monitor progress toward housing and personal stability goals.
• Connect clients with housing resources, healthcare providers, behavioral health services, employment programs, financial assistance, and other community resources.
• Assist clients with completing applications, obtaining identification documents, benefits enrollment, and accessing supportive services.
• Advocate on behalf of clients with landlords, service providers, government agencies, and community organizations.
• Maintain accurate, timely, and confidential case documentation in accordance with organizational and funding requirements.
• Participate in interdisciplinary team meetings and collaborate with internal staff and external partners to coordinate services.
• Conduct outreach as needed to engage clients and promote continued participation in services.
• Monitor client progress, adjust service plans as appropriate, and support successful transitions into stable housing.
• Maintain knowledge of local community resources, housing programs, and applicable regulations.
