Responsibilities include initial assessments, referrals, confidential record-keeping, data entry, and collaboration with partners.

Qualifications

• High School Diploma or GED required.
• 1 year direct service or Case Management experience required.
• Spanish bilingual is a plus.
• Knowledge of community resources and ability to foster client relationships.
• Excellent written and oral communication skills.

Responsibilities

• Assess client needs to determine appropriate referrals.
• Provide referrals to housing, employment, healthcare, legal, and other services.
• Prepare and maintain confidential case records and DAP notes.
• Enter client data into electronic database/portal.
• Collaborate with service providers on resource-related issues.
• Assist clients with securing mental health and substance use resources.
• Support clients in obtaining identifying documents and basic money skills.
• Assist with housing opportunities and discharge information.
• Transport clients as needed and participate in case staffing.
• Perform other duties to support clients.
