Job summary

Jobot is seeking a Case Manager (Community Health) in Albuquerque, NM. This on-site, permanent role supports individuals and families through case management, care coordination, and connection to community resources within a federally qualified health center.

Responsibilities include assessing barriers to care, coordinating with medical and behavioral health teams, and maintaining documentation in the electronic health record.

Qualifications

• Previous case management, care coordination, peer support or community-service experience.
• Experience working with individuals, families and underserved populations.
• Certified Peer Support Worker (CPSW) preferred.
• Bachelor’s degree preferred.
• Backgrounds in Peer Support, CSW, CCSS, community health, behavioral health or social services are encouraged.

Responsibilities

• Provide ongoing case management and care coordination for an assigned caseload.
• Assess patient needs and identify medical, behavioral health and social barriers to care.
• Connect patients with community resources and support programs.
• Coordinate referrals for housing, transportation, food assistance, behavioral health and other social services.
• Assist patients with navigating healthcare and community-based systems.
• Develop and maintain individualized service or care plans.
• Follow up on referrals and services to help maintain continuity of care.
