Qualifications

• Bachelor's degree preferred; equivalent experience considered.
• Experience in case management or social services preferred.
• Strong knowledge of community resources and systems.
• Excellent communication, interpersonal, and crisis intervention skills.
• Ability to manage multiple cases with confidentiality.
• Valid driver's license and reliable transportation may be required.

Responsibilities

• Conduct client intake assessments and develop individualized case management plans.
• Maintain ongoing contact with clients to monitor progress toward housing and personal stability goals.
• Connect clients with housing resources, healthcare providers, behavioral health services, employment programs, financial assistance, and other community resources.
• Assist clients with completing applications, obtaining identification documents, benefits enrollment, and accessing supportive services.
• Advocate on behalf of clients with landlords, service providers, government agencies, and community organizations.
• Maintain accurate, timely, and confidential case documentation in accordance with organizational and funding requirements.
• Participate in interdisciplinary team meetings and coordinate services with internal and external partners.
• Conduct outreach to engage clients and promote continued participation in services.
• Monitor client progress, adjust service plans as appropriate, and support transitions into stable housing.
• Maintain knowledge of local resources, housing programs, and applicable regulations.
