  • Maintain accurate, timely, and confidential case documentation in accordance with organizational and funding requirements.
  • Participate in interdisciplinary team meetings and coordinate services with internal and external partners.
  • Conduct outreach to engage clients and promote continued participation in services.
  • Monitor client progress, adjust service plans as appropriate, and support transitions into stable housing.
  • Maintain knowledge of local resources, housing programs, and applicable regulations.

Job Description

Our client is seeking a compassionate and motivated Case Manager to provide comprehensive support services to individuals experiencing homelessness. This role is responsible for assessing client needs, developing individualized service plans, connecting clients with housing and community resources, and advocating for clients as they work toward self-sufficiency and permanent housing.

The ideal candidate is empathetic, highly organized, and comfortable working with individuals facing complex challenges, including mental health concerns, substance use, unemployment, and housing instability.

Responsibilities

  • Excellent communication, interpersonal, and crisis intervention skills.
  • Ability to establish trust and build rapport with individuals from diverse backgrounds.
  • Strong organizational skills with the ability to manage multiple cases simultaneously.
  • Experience with electronic case management systems and Microsoft Office.
  • Ability to maintain confidentiality and exercise sound professional judgment.
  • Valid driver's license and reliable transportation may be required.
