Job summary

Jobot is seeking a detail-oriented Case Manager – Community Health (FQHC) in the Albuquerque, NM area. You will coordinate care, connect patients to community resources, and advocate to improve health outcomes with medical, behavioral health, and social services teams.

The role centers on social determinants of health such as housing, transportation, and food access. It offers a stable schedule, team collaboration, and weekly pay with benefits eligibility (75% covered).

Qualifications

• 2+ years of case management or care coordination experience.
• Preferred CPSW certification or a related bachelor’s degree.
• Experience with underserved or vulnerable populations.
• Knowledge of New Mexico community resources is a plus.
• Strong communication, organizational, and patient advocacy skills.
• Experience with EHR and care coordination tools is preferred.

Responsibilities

• Provide case management services including care coordination, referrals, and resource navigation.
• Conduct patient assessments to identify medical, behavioral, and social needs.
• Connect patients with housing, transportation, food programs, and behavioral health support.
• Collaborate with clinical and behavioral health teams to develop care plans.
• Maintain documentation in the EHR and track patient progress.
• Support patient engagement, education, and follow-up to improve outcomes.
