Job summary

Sulzbacher Center is seeking a Case Manager to help clients achieve housing stability, medical and financial goals, and self-sufficiency. This role collaborates with teams to navigate transportation, health services and resource applications, advocating against barriers to homelessness.

As the primary staff contact for residents, you will promote respectful interactions, document progress, and coordinate access to services, supports, and trainings across departments.

Qualifications

• Bachelor's Degree in Social Work or other helping profession.
• Ability to establish cooperative working relationships with clients, colleagues and volunteers.
• Excellent interpersonal skills and ability to work independently in a fast paced environment.
• Ability to manage crisis situations.
• If driving a company vehicle on work-related business, must be 21 years of age with acceptable MVR.
• Ability to pass a Level II Background Screening.

Responsibilities

• Works as a team with the Men's Team and other case managers to assess clients’ needs and access resources.
• Increases clients’ awareness of available community and on-site services and assists access to those resources.
• Maintains confidential progress notes and documents client plans.
• Develops partnerships with community agencies to serve homeless clients.
