Formación

• Six years of experience in case management.
• IMHATT certificate within three months of hire date.
• Mandated reporter training/certificate.
• Trauma Informed Care training.
• Team oriented with strong multitasking and organizational skills.

Responsabilidades

• Provide targeted case management services to children, youth and adults with mental health illnesses and substance abuse disorders and their families through access and care coordination.
• Gather enrollment consents, screenings, risk assessments, plan of care updates, and notes per policy.
• Communicate aims and goals clearly to patients, community members and staff.
• Participate in quality improvement activities with the clinical team.
• Complete data reports for program reviews.
• Identify new housing sources and engage with homeless populations and shelters.
