Job summary

Acacia Network is seeking a Case Worker in Queens, NY to implement, monitor, and evaluate case management activities. You will work directly with individuals facing homelessness and coordinate health care referrals, paperwork processing, and access to social services to stabilize housing and lives.

Under supervision of the Program Director, you will assess needs, help secure permanent housing, benefits, and services, and maintain strong relationships with public agencies.

Qualifications

• Associate's degree required; high school diploma with relevant experience acceptable.
• 2-year experience in case management or social/human services preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct intake interviews and assessments with clients.
• Counsel clients facing mental health, addiction, or abuse issues.
• Assist clients in obtaining housing, healthcare, and benefits.
• Maintain and update case files and client records in DFTA systems.
• Network with community partners and attend required meetings.
