Job summary

Acacia Network is seeking a dedicated Case Manager in Brooklyn, NY for a regular full-time role. You will provide case management and housing support to clients, maintain documentation, and ensure compliance with program requirements.

Qualifications

• BA degree in relevant field; Masters preferred.
• Minimum of 2 years of case management experience.
• Experience working with homeless population(s).
• Housing knowledge and familiarity with community resources.
• Excellent oral and written communication.
• Ability to document and report monthly progress.

Responsibilities

• Provide case management and on-site crisis intervention.
• Complete required service plans and housing applications.
• Educate participants on budgeting, home-safety, and landlord-tenant relations.
• Document tenant contacts and progress notes.
• Coordinate housing-related workflows (Section 8, NYCHA).
• Refer to supportive services and advocate with government agencies.
