Job summary

Acacia Network in Brooklyn, NY is seeking a Case Manager Supervisor to provide case management and housing support to families, under the Director of Social Service. You will also ensure monthly reporting to maintain program compliance and assist clients toward independence.

Your duties include conducting needs assessments, home visits, coordinating Section 8/Housing applications, maintaining records, and advocating with agencies.

Qualifications

• BA degree in relevant field; Masters degree preferred.
• Minimum of 2 years of Case Management Experience.
• Experience working with homeless population(s).
• Housing Knowledge.
• Familiarity with community Resources.
• Excellent oral and written communication.

Responsibilities

• Conducts needs assessment and case management for assigned families.
• Conducts intakes, home visits and apartment inspections.
• Holds bi-weekly interviews with clients.
• Maintains up to date files including client history and profile.
• Provides information and referrals, arranges for provision of services, advocates and intervenes with government agencies, short-term counseling and assists in developing service plan with goal of restoring family independence.
• Coordinates workshops, seminars and other activities for general client population.
• Performs other related duties as assigned.
• Makes recommendations to improve level of services.
