Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Colorado driver’s license required within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Coordinate services and resources; monitor client progress.
• Provide advocacy and crisis intervention as needed.
• Maintain care plans and ensure documentation in the EHR.
• Collaborate with community partners and health providers.
• Assist clients with applying for benefits and housing resources.
