Job summary

Mid-Willamette Valley Community Action Agency is seeking a dedicated Case Manager to assess families, develop individualized plans, and connect them with housing resources and supportive services at the ARCHES Family Day Center in Salem. You will coordinate with program staff and community partners to meet resident needs and track progress in the data system.

The role emphasizes trauma-informed practices, empathy, and strong organizational skills, with on-site work Monday–Friday.

Qualifications

• High School Diploma or GED is required.
• Four years of relevant social services experience are required.
• Knowledge of trauma-informed care and working with vulnerable populations is preferred.

Responsibilities

• Meets with families to perform assessments to determine needs, strengths, and resources.
• Develops individualized goals and action plans for families.
• Develops collaborative crisis response plans for families.
• Identifies resources to support families and connects them to services.
• Maintains detailed case notes and accurate data in the system.
