Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to provide on-site case management for low acuity clients. You will coordinate services, perform screenings, and develop action plans to meet clients’ needs, while acting as an advocate within community and social service systems.

Applicants should have HS/GED (VOALA training within 6 months) or a Bachelor’s Degree in Social Services, and must pass fingerprint clearance and background checks.

Qualifications

• Fingerprint clearance and background check required.
• Ability to work under emotional stress and with challenging clients.
• HS/GED required; VOALA Case Management training within 6 months of hire.

Responsibilities

• Conduct comprehensive screenings and assessments; collect functional, environmental, psychosocial, financial, employment, housing, educational, and health information.
• Develop individualized case plans; coordinate services and resources to meet needs.
• Coordinate case conferences; monitor progress and advocate for clients with agencies and service providers.
• Provide linkage to services and support systems to ensure needs are met.
• Engage in outreach, collaboration, and on-site program delivery as required.
