Job summary

IPMG is now happy to offer a sign on bonus of $500 within your first 6 months with us and is 100% employee owned through an ESOP. This full-time, hybrid Case Manager role requires travel to community-based meetings multiple times per week and working remotely from a home office.

Qualifications

• Bachelors degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelors degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• A valid drivers license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education about the Medicaid waiver process
• Create an Individual support plan based on strengths, needs, goals, and desires of Individuals
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

What You’ll Do:

• Provide information and education to help Individuals understand the Medicaid waiver process

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:
