Job summary

IPMG is seeking a Case Manager in Indiana for a full-time, hybrid role that requires travel to community-based meetings and working remotely from a home office.

Applicants must live in Indiana or within 10 miles of the state line and be able to participate in in-person meetings as needed. The position offers an ESOP company structure and sign-on bonus within 6 months.

Qualifications

• Bachelor's degree in psychology, sociology, social work or similar human services field is required.
• Bachelor's degree in an unrelated field with 1 year direct experience with individuals with intellectual/developmental disabilities, or 2 years with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet, and smartphone.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

What You’ll Do:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.

• A valid driver's license, car insurance, and reliable transportation
