Job summary

IPMG, an Employee Owned organization, is hiring for a Case Manager in Indiana. This full-time, hybrid position requires travel to community-based meetings multiple times per week and working remotely from a home office.

Applicants must reside within Indiana or within 10 miles of the state line due to in-person meetings. IPMG offers a comprehensive program including an after-hours crisis line, ongoing training, opportunities for advancement, generous PTO/holidays, and mileage reimbursement.

Qualifications

• Bachelor’s Degree in Human Services required.
• Professional experience with individuals with intellectual and developmental disabilities preferred.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

What You’ll Do

Who You’ll Work With

• Case managers work with Individuals on two Medicaid Waivers; Family Supports Waiver (FSW) and Community Integrated Habilitation Waiver (CIH)

What You Need

• A valid driver's license, car insurance, and reliable transportation
• A Bachelor’s Degree in Human Services and a minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
• Designated home office setting with computer, internet service, and smart phone
