Job summary

Indiana Professional Management Group, Inc (IPMG) is seeking a Case Manager for a full-time hybrid role based in Indiana. The position requires travel to community-based meetings several times per week and work from a home office.

Applicants must reside in Indiana or within 10 miles of the state line and have a valid driver’s license with reliable transportation. Responsibilities include educating Individuals about Medicaid waivers, creating tailored support plans, assisting access to services,

Qualifications

• Bachelor’s Degree in Human Services and at least one year of professional experience working with Individuals with intellectual and developmental disabilities (IDD) is required.
• A valid driver’s license and reliable transportation are required.
• Designated home office with computer, internet, and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the Individuals served.
• Help Individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and coordinate with service providers.

What You’ll Do

• Provide information and education to help Individuals understand the Medicaid waiver process

Who You’ll Work With

• Case managers work with Individuals on two Medicaid Waivers; Family Supports Waiver (FSW) and Community Integrated Habilitation Waiver (CIH)

What You Need

• A valid driver’s license, car insurance, and reliable transportation
• A Bachelor’s Degree in Human Services and a minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
