Job summary

Indiana Professional Management Group, Inc. is seeking a qualified Case Manager for a full-time hybrid role. The position requires travel to community-based meetings several times per week and a home-office setup for remote work.

Applicants must reside in Indiana or within 10 miles of the state line and have a Bachelor’s in Human Services with at least one year of relevant experience with individuals with IDD. IPMG offers ESOP and a sign-on bonus of $500 within six months.

• Valid driver’s license, car insurance, and reliable transportation.
• Designated home office with computer, internet and smartphone.

Responsibilities

• Provide information to help individuals understand Medicaid waivers.
• Create an individualized support plan based on the individuals' strengths, needs, goals and desires.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plans with service providers.

What You’ll Do

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community‑based team meetings to review plan of care and collaborate with service providers as needed

Who You’ll Work With

• Case managers work with Individuals on two Medicaid Waivers; Family Supports Waiver (FSW) and Community Integrated Habilitation Waiver (CIH)

What You Need

• A valid driver’s license, car insurance, and reliable transportation
• A Bachelor’s Degree in Human Services and a minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
• Designated home office setting with computer, internet service, and smart phone
