Job summary

IPMG is seeking a Case Manager in Indiana to provide information on Medicaid waiver processes, develop individualized support plans, and help individuals access necessary services. This full-time, hybrid role requires travel to community meetings several times per week and some remote work from a home office.

Applicants must reside in Indiana or within 10 miles of the state line and attend in-person meetings.

Qualifications

  • Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
  • Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
  • Valid driver's license, car insurance, and reliable transportation.
  • Ability to pass thorough background screens.
  • Designated home office setting with computer, internet service, and smartphone.

Responsibilities

  • Provide information and education to help Individuals understand the Medicaid waiver process.
  • Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
  • Help Individuals access services and supports they need to meet their goals.
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field
Bachelor's degree in an unrelated field with 1 year direct experience with people with disabilities or 2 years with aging population

Let’s talk about IPMG’s Case Manager position! This full-time, hybrid position, requires travel to community-based meetings multiple times per week, and requires working remotely from a home office setting.

This position requires in-person meetings; therefore, applicants must live within the State of Indiana or within 10 miles of the state line.

What You’ll Do:

Who You’ll Work With:
  • Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)
What You Need:
  • A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

  • A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
