Job summary

Indiana Professional Management Group (IPMG) is seeking a Case Manager in Lebanon, IN for a full-time hybrid role. You will provide education on Medicaid waivers, craft individualized support plans, and help Individuals access essential services to meet their goals.

The position requires travel to community-based meetings several times per week and in-person work when needed, with remote work from a home office.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• A valid driver's license, car insurance, and reliable transportation.
• The ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smart phone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Let’s talk about IPMG’s Case Manager position! This full-time, hybrid position, requires travel to community-based meetings multiple times per week, and requires working remotely from a home office setting.

This position requires in-person meetings; therefore, applicants must live within the State of Indiana or within 10 miles of the state line.

Who You’ll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
