Job summary

IPMG is hiring a Case Manager in a full-time hybrid role requiring travel to community-based meetings and occasional remote work from a home office. Applicants must reside in Indiana or within 10 miles of the state line and be able to attend in-person meetings.

The role includes educating individuals about Medicaid waivers, developing personalized support plans, coordinating services, and leading team meetings with providers.

Qualifications

  • Bachelor's degree in psychology, sociology, social work, education, nursing, or related human services field.
  • Bachelor's degree in an unrelated field plus 1 year of direct experience with individuals with disabilities or 2 years with aging population.
  • Must pass thorough background screens.
  • Dedicated home office with computer, internet, and smartphone.

Responsibilities

  • Provide information and education to help individuals understand the Medicaid waiver process.
  • Create an individual support plan based on individuals' strengths, needs, goals, and desires.
  • Help individuals access services and supports to meet their goals.
  • Facilitate regular community-based team meetings to review plans of care with service providers.

Let's talk about IPMG's Case Manager position! This full-time, hybrid position, requires travel to community-based meetings multiple times per week, and requires working remotely from a home office setting.

This position requires in-person meetings; therefore, applicants must live within the State of Indiana or within 10 miles of the state line.

What You'll Do:

  • Provide information and education to help Individuals understand the Medicaid waiver process
  • Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
  • Help Individuals access services and supports they need to meet their goals
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Who You'll Work With:

  • Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:

  • A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
  • A Bachelor's degree in an unrelated fieldand a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
  • A valid driver's license, car insurance, and reliable transportation
  • The ability to pass thorough background screens
  • Designated home office setting with computer, internet service, and smart phone
