Job summary

PAM Health Rehabilitation Hospital of Dover is seeking a Case Manager to coordinate interdisciplinary care for inpatient populations. The role involves reviewing admission records to ensure proper resource use and determining appropriate level of care, assessing discharge needs, and coordinating discharge plans with the team.

You will work with nurses, therapists, social workers and other professionals to improve financial and quality outcomes, while maintaining payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for a designated patient population across inpatient records to ensure proper utilization of hospital resources and determination of admission for appropriate level of care.
• Assesses and identifies discharge needs and coordinates appropriate discharge plan.
• Work collaboratively with the interdisciplinary team to facilitate achievement of desired financial and quality outcomes.
• Maintain and enhance payor relationships.
