You will work with the interdisciplinary team to achieve desired financial and quality outcomes and maintain/payor relationships.

Qualifications

• Interdisciplinary coordination of care for a designated patient population is required.
• Current BLS certification is required.
• Licensure in the state where the hospital resides is preferred.

Responsabilités

• Coordinate care across disciplines to ensure appropriate level of care.
• Review inpatient admission records for proper utilization of resources.
• Assess discharge needs and coordinate the discharge plan.
