Job summary

PAM Health Rehabilitation Hospital of Tulsa is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population and to review admission records for appropriate level of care. You will assess discharge needs and coordinate discharge planning, while maintaining payor relationships and supporting the team to achieve financial and quality outcomes.

Candidates should have state licensure (preferred) and a current BLS certification, with three to five years of inpatient

Qualifications

• Licensure in the state where the hospital resides is preferred.
• BLS certification required.
• Three to five years of inpatient experience.

Responsibilities

• Coordinate interdisciplinary care for a designated patient population.
• Review inpatient admission records to ensure proper utilization of hospital resources and determine admission level of care.
• Assess discharge needs and coordinate discharge planning with the interdisciplinary team.
• Maintain and enhance payor relationships and collaborate with the team to achieve financial and quality outcomes.
