Job summary

PAM Health Rehabilitation Hospital of Georgetown is seeking a Case Manager to coordinate care across disciplines for an inpatient population, reviewing admission records to ensure appropriate level of care and supporting discharge planning.

You will work with the interdisciplinary team to improve financial and quality outcomes and maintain relationships with payors. Licensure in the state and BLS certification are required.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for the designated patient population as part of an interdisciplinary team.
• Review inpatient admission records to ensure proper utilization of hospital resources and admission decisions.
• Assess discharge needs and coordinate the appropriate discharge plan.
• Work with the interdisciplinary team to facilitate financial and quality outcomes; maintain payor relationships.
