Qualifications

Job description

Jewish Community Services (JCS) is seeking a compassionate, organized, and client-focused Case Manager to provide direct case management services to individuals and families seeking support and resources. This full‑time role is ideal for someone who enjoys building meaningful relationships, advocating for clients, coordinating services, and helping individuals achieve greater independence and economic stability. You will work directly with clients to assess needs, develop service plans, connect clients with community resources, monitor progress, and support successful outcomes. This position operates in a hybrid work environment.

Primary Responsibilities
  • Provide individualized case management services in accordance with agency and grant requirements.
  • Conduct client assessments and develop individualized service and review plans.
  • Establish and maintain advocacy relationships with clients.
  • Monitor client progress, evaluate outcomes, and determine when goals have been achieved.
  • Promote, support, and encourage client independence and self-sufficiency.
  • Coordinate services and collaborate with community organizations and service providers.
  • Research and identify available resources to address client and community needs.
  • Make appropriate referrals and support clients through the referral process.
  • Maintain accurate, timely, and complete documentation in accordance with agency policies and funding requirements.
  • Participate in Total Performance Management (TPM) activities, including quality improvement, customer satisfaction, service outcomes, efficiency, and risk management initiatives.
  • Contribute to program development, agency events, and new service initiatives.
  • Stay informed about developments, best practices, and regulatory requirements within the human services field.
  • Participate in meetings, planning discussions, and special projects as needed.
Qualifications & Experience
  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field required; Master's degree preferred.
  • Two or more years of case management or related human services experience preferred.
  • Knowledge of community resources and the ability to connect clients with appropriate supports and services.
  • Experience conducting assessments, developing service plans, and maintaining client documentation.
  • Understanding of financial assistance programs and budgeting resources preferred.
  • Strong interpersonal, written, verbal, and organizational skills.
  • Ability to work independently, exercise sound judgment, and manage multiple priorities.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Familiarity with and sensitivity to Jewish traditions, culture, and values a plus.

Pay: $55,000-$59,000/year
