The Staten Island Jewish Community Center is seeking a dedicated Case Manager to support OPWDD- and regulatory-based services, coordinating intake, enrollment, and a range of habilitation and community-based opportunities. The role collaborates with the Department Senior Director to ensure timely enrollment, reporting, and compliance for current and future programs.

Qualifications

• Must have a valid NY State Driver’s License.
• Experience within a human service-related environment.
• 2 years’ experience working with the Special Needs Population.
• Knowledge of OPWDD Policies.

Responsabilités

• Oversees client care by assessing treatment needs during intake and enrollment, developing and monitoring individualized plans, coordinating with interdisciplinary teams.
• Manages admission of new clients by reviewing records and conducting orientations.
• Determines client needs through intake interviews and coordinating evaluations and care plans.
• Coordinates treatment programs by developing schedules, arranging resources including transportation.
• Monitors ongoing cases; evaluates treatment effectiveness and advocates for services and entitlements.
• Maintains accurate and compliant client records and progress updates.
• Communicates client progress through weekly meetings and formal evaluations.
• Facilitates client transition by strengthening transition plans and coordinating post-transition services.
• Improves treatment outcomes by analyzing processes and updating policies and procedures.
• Prepares comprehensive reports by collecting and summarizing treatment outcomes and statistics.
• Maintains professional expertise through continuing education and networking.
• Strengthens department and organizational reputation by pursuing new initiatives.
