Jewish Community Services (JCS) in Baltimore, MD, is seeking a compassionate Case Manager to provide direct case management to individuals and families. This full-time, hybrid-position involves assessing needs, developing service plans, linking clients with resources, monitoring progress, and supporting outcomes.

The role requires a Bachelor's degree in a related field (Master's preferred) and 2+ years of experience.

Qualifications

• Bachelor's degree required; Master's preferred.
• Two+ years of case management or related human services experience preferred.
• Knowledge of community resources and ability to connect clients with supports.
• Experience conducting assessments, developing service plans, and documenting.
• Familiarity with Jewish traditions, culture, and values a plus.

Responsibilities

• Provide individualized case management per agency and grant requirements.
• Assess client needs and develop service and review plans.
• Establish and maintain advocacy relationships with clients.
• Monitor progress, evaluate outcomes, and determine goal achievement.
• Promote client independence and self-sufficiency.
• Coordinate services and collaborate with community organizations.
• Research and identify available resources and make referrals.
• Maintain accurate documentation per policies and funding requirements.
• Participate in TPM activities and program development.
• Stay informed about developments in human services field.
• Participate in meetings and special projects as needed.
