Stanford Health Care in Palo Alto, CA seeks a Clinical Case Manager (RN) to coordinate care and discharge planning for a caseload across inpatient units. You will lead utilization review, review charts for timeliness and appropriate services, and collaborate with physicians, social workers and the care team to ensure efficient, high-quality care.

Licensed RN in California with at least 3 years of related experience is required; a Bachelor's degree is required and a Master's degree is preferred.

Qualifications

• Bachelor's Degree from an accredited college or university required
• Master's Degree preferred
• Three (3) years of progressively responsible and directly related work experience required

Responsabilités

• Coordination of Care — manages each patient's transition through the system and discharge.
• Discharge Planning — coordinates timely implementation of discharge plans with the team.
• Utilization Review — reviews inpatients for admission level of care and length of stay.
• Education/Consultation — educates patients, families, and staff on discharge planning.
• Lead Work — may lead administrative/clinical support staff and participate in committees.
• Patient Assessment / Plan of Care — collaborates to assess and plan care.
