Job summary

Community Health Network is seeking a Case Manager to oversee assessment, planning, and coordination of care across the continuum. You will work with the care team to ensure smooth transitions between levels of care and to support discharge planning.

Required are an Indiana-licensed social work credential (LCSW/LSW) and a Master’s in Social Work, plus 2+ years in inpatient, outpatient, or home-based social work, with strong communication and problem-solving skills.

Qualifications

• Master's Degree in Social Work is required.
• LCSW or LSW in Indiana is required.
• Excellent communication skills are essential.
• Ability to collaborate in a team setting.
• Positive attitude toward problem-solving is expected.

Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate services across the continuum of care.
• Support the healthcare team toward a smooth transition between levels of care.
• Coordinate discharge planning and address barriers or delays.
• Document patient records accurately and timely in the transition to quality software.
• Collaborate with nurse case managers on discharge planning.
• Demonstrate awareness of patient rights and hospital policies.
• Participate in staff development activities and care management meetings.

Education

Master's Degree in Social Work
