Job summary

View Behavioral Health LLC in Long Beach, CA is seeking a Case Manager/Discharge Planner to coordinate discharge planning for patients and collaborate with the treatment team.

Under the supervision of the Director of Social Work, you will assess patient needs, arrange housing, referrals, and bridge hospital to community care, with opportunities to support IOP/PHP as needed. The role requires a bachelor’s degree, 2+ years’ experience, English proficiency, and CPR/BLS; 8-hour day shifts, Mon-Fri.

Qualifications

• Bachelor’s degree in psychology, human services, social work, or counseling is required.
• Two+ years direct service experience with severely and persistently mentally ill patients in inpatient or outpatient settings.
• Capable of speaking English, reading, and writing clearly.
• CPR/BLS/First Aid certification or ability to obtain within 90 days of employment.

Responsibilities

• Coordinate discharge plans with the patient’s treatment team.
• Provide referrals for placement, case management, and aftercare services.
• Bridge hospital and referral sources to ensure smooth care transitions.
• Maintain up-to-date knowledge of community resources.
• Assist with benefits, admissions, and discharge steps with intake staff.
• Collaborate with social services and physicians to ensure continuous care.
