Job summary

UCSF Health is seeking a Case Manager to coordinate resources and services for patients, conduct utilization reviews, and oversee discharge planning across a CASeload. You will participate in program development, quality initiatives, and lead daily huddles with the multidisciplinary team.

The role requires a Nursing license, healthcare experience, and strong communication skills, with flexibility to operate across UCSF Medical Center locations.

Qualifications

• Bachelor's Degree in Nursing is required.
• Minimum of two years of healthcare experience.
• One year of case management experience.
• Strong written and oral communication, diplomacy, organizational and analytic skills.
• Self-directed, assertive and creative in problem solving.
• Proficient with an electronic patient record system.
• Ability to work collaboratively with interdisciplinary teams.
• Flexibility to orient and work at all UCSF Medical Center locations.

Responsibilities

• Coordinate resources and services in response to patient needs and fiscal realities.
• Perform utilization review activities and oversee discharge planning for assigned caseload.
• Contribute to team goal setting as a member of the interdisciplinary team.
• Serve as a consultant to clinical teams and other departments; participate in program development and quality improvement initiatives.
• Provide age-appropriate assessments, interpretation of data, and delivery of interventions.
• Lead daily huddles and Multidisciplinary Rounds; conduct complex case reviews; support quality and documentation audits.
• Help manage daily departmental operations and transition planning to ensure safe and timely discharges.
