Advocates in Boston, MA is seeking a Case Manager to document and maintain program participants’ records and plans, focusing on advocacy, empowerment, and community integration. The role includes developing ISP goals, coordinating health care, and linking clients to housing and benefits.
A client-centered, inclusive approach is essential. The position requires strong communication and organizational skills, a valid driver’s license, and the ability to work with diverse populations.
Starting Rate: $21.50/hour
Schedule: Monday through Friday 8am-4pm or 9am-5pm
(The hours can be flexible and on the weekend as needed)
The Case Manager is responsible for documentation and maintenance of program participants’ records and plans and includes all areas of the Individual Service Plan (ISP), medical concerns and appointments. This role will focus on individual advocacy, empowerment, and community integration.
