Job summary

You will collaborate with health, education, and social service providers, conduct assessments, document progress, and ensure compliance with funder and program requirements.

• Linguistically and culturally competent to holistically serve the foster youth populations.

Responsibilities

• Provide quality and timely client-centered services to all program participants.
• Maintain regular contact to ensure client needs are met and progressing towards self-sufficiency.
• Comply with funder, program, and organization requirements.
• Work closely with health, education, and social service providers to provide holistic case management.
• Complete individualized assessments of clients addressing short- and long-term needs.
• Document and track client progress, case notes, and required data in systems.
• Attend and participate in staff meetings for training and program planning.
• Participate in regular supervision meetings to discuss cases and progress.
• Utilize resources to address difficult client situations.
• Collaborate with employment and education staff to ensure holistic care.
• Demonstrate respect for all people, families, and communities.
• Ensure adherence to service provision standards and program outcomes.
• Other duties as assigned.
