Job summary

Lutheran Family and Children’s Services (LFCS) in Missouri is seeking a Care Coordinator to deliver client-centered care, including case management, pregnancy counseling, parenting and adoption education, and community outreach across the state.

You will develop individualized service plans with clients, counsel and teach skills in home or virtual settings, and respond to crises within program guidelines to promote safety and stability for families.

Qualifications

• Bachelor's or Master's Degree in Social Work, Counseling, Psychology, Criminal Justice, or other Human Behavior Science.

Responsibilities

• Coordinate and provide care to clients that are safe, timely, effective, efficient, equitable, and client-centered.
• Provide case management, pregnancy counseling, parenting and adoption education, community education and outreach, and post-adoption support for birth parents.
• Create a service plan in conjunction with the client to develop goals and objectives that are consistent with what the client wishes to accomplish.
• Counsel, advocate, and teach clients the skills identified in their service plan face to face in home or virtually.
• Respond to client’s crisis situations within program parameters in a timely manner according to program protocols.
