Job summary

Lutheran Family Services is hiring a Case Manager for Dana Village in Fremont, NE. The role provides direct services to residents, including those in the Root + Rise program, to help them achieve self-sufficiency.

The position requires strong communication, excellent organizational skills, and the ability to coordinate with health and social service partners. Some evenings may be required; a valid driver’s license is needed where applicable.

Qualifications

• Bachelor’s degree in Human Services or related field preferred.
• High school diploma or GED required.
• Experience in case management with diverse populations preferred.
• Bilingual English/Spanish preferred but not required.

Responsibilities

• Provide direct case management services to Dana Village residents, including Root + Rise participants.
• Maintain regular contact to address client needs and progress toward self-sufficiency.
• Coordinate with health, education and social service providers for holistic care.
• Complete individualized assessments addressing short- and long-term needs.
• Document progress and data in appropriate systems in a timely manner.
• Attend staff meetings for training and program planning; participate in supervision.
• Collaborate with employment and education staff to support client outcomes.
• Uphold program standards, funder requirements, and organizational values.
