Resources for Human Development (RHD) is seeking a compassionate Case Manager in the Cedar Rapids area to coordinate services with participants, families, and providers. You will work on community-based case management, advocacy, and documentation while promoting participant choice and recovery goals.

The role emphasizes teamwork, ongoing professional development, and exposure to a broad range of behavioral health and social services.

Qualifications

• Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, Counseling, or related field.
• One year of Case Management experience preferred.
• Valid Driver's License and ability to travel within service area.

Responsibilities

• Coordinate services with participants, families, clinical staff, medical providers, and community partners.
• Support holistic, strengths-based approach to service delivery and participant self-determination.
• Complete assessments, service plans, progress notes, and reports in a timely manner.
• Advocate for participant access to needed services and resources.
