Qualifications

• Bachelor’s degree or higher preferred in psychology or related field.
• Two years of experience or training in Human Services acceptable.
• Experience with case management and crisis management preferred.
• Knowledge of diverse communities and ability to work across backgrounds.
• Valid driver’s license, reliable vehicle, and daily local travel required.

Responsibilities

• Develop with clients integrated treatment team plans, updated every 3 months or sooner as needed.
• Facilitate implementation of clients’ goals: community living, daily living, skills, medication management, health care, and services.
• Discuss discharge goals with client and develop a discharge plan.
• Conduct required assessments.
• Arrange transportation to appointments to support the treatment plan.
• Develop crisis response and safety plans; assess safety as needed.
• Coordinate activities with other agencies and providers.
• Advocate for the client and partner with service providers.
• Maintain caseload and meet monthly program billing.
• Attend meetings, training and supervision sessions.
• Document all services and communications per guidelines.
• Participate in PQI activities and uphold HIPAA confidentiality.
• Other duties as assigned by supervisor.
