Job summary

Community Support Services of Missouri (CSS) is seeking a dedicated Case Manager in Joplin, MO to coordinate services for individuals with developmental disabilities. You will interview clients and families, develop Person-Centered Service Plans, monitor progress, and collaborate with interdisciplinary teams to arrange needed supports.

The role requires strong knowledge of case management, DMH policies, and the ability to maintain accurate records.

Qualifications

Strong knowledge of case management methods, principles, and techniques.
Strong knowledge of developmental disabilities and services available for clients.
Experience with interviewing methods and techniques.
Ability to manage a caseload of clients with developmental disabilities.
Ability to analyze information to inform service plans.
Ability to develop practical service plans for persons with developmental disabilities.
Ability to negotiate and communicate diplomatically.
Ability to lead and inspire colleagues toward the agency's goals.
Excellent communication skills.

Responsibilities

Interview person served, families, and others to gather data and schedule assessments.
Develop PCSP with interdisciplinary teams and attend team meetings.
Monitor PCSP per DMH Directive.
Complete tasks in SetWorks with 90% accuracy.
Attend staff meetings and review policies and procedures.
Complete required training and new processes for DMH.
Log hours within 3 working days.
Process service authorizations and budgets on time.
Maintain CIMOR and SetWorks data.
Represent the agency at DMH workgroups and committees.
Attend DMH Due Process meetings as needed.
Assist in interview, orientation and training of new employees.
Participate in QA and CARF outcomes.
