The Center for Human Services is seeking an Interim Case Coordinator in St. Louis, MO. The role supports individuals with developmental disabilities and their families to achieve independence and community inclusion, with temporary caseload coverage.

Position requires driving to homes for face-to-face visits and coordinating services with a multi-disciplinary team. Hybrid/remote options may apply; requires living in the service area.

Qualifications

• Bachelor’s degree or RN license is required.
• Reliable transportation and driving record required.
• 1+ year experience with IDD in community setting or related field.
• Experience with case management and documentation software.

Responsabilités

• Assist individuals with developmental disabilities to meet goals.
• Advocate, monitor, and evaluate services for individuals and families.
• Maintain documentation within three business days of activities, ensure billable hours.
• Develop action plans and coordinate with teams and community resources.
