Job summary

PACS Group is seeking a qualified nurse/care coordinator to develop and implement comprehensive care plans for residents. You will work with residents, families, and healthcare providers to ensure high-quality, cost-conscious care.

The role requires clinical nursing experience, familiarity with Medicare/Medicaid programs, and strong communication skills to coordinate with physicians, staff, and payers. CCM certification is a plus.

Qualifications

• Bachelor's Degree in Nursing or Social Work; RN license preferred.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• CCM certification a plus.
• Must maintain continuing education/licensing.

Responsibilities

• Communicate regularly with residents and family about Plans of Care and treatment protocols.
• Attend and participate in morning meetings to facilitate team communication.
• Answer residents' questions about care, treatment plans, or illness progression.
• Monitor and adjust residents' statuses based on changing needs.
• Organize and prioritize daily work and charting.
• Complete documentation as required.
