Job summary

PACS is seeking a qualified healthcare professional to coordinate resident care and manage plans of care with families, providers, and payers. You will oversee admissions, discharges, and referrals while ensuring cost-effective, high-quality care.

The role requires strong communication, organization, and computer skills within a fast-paced environment, with emphasis on resident-centered care and teamwork across departments.

Qualifications

• Bachelor's degree in Social Work, Human Services, Nursing, or related field preferred.
• Prior healthcare/case management or social services experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• Familiarity with PCC (PointClickCare) or other EMR systems preferred.
• Certification as a CCM a plus.

Responsibilities

• Communicate regularly with residents and families about Plans of Care, PT, OT, and other treatment protocols.
• Attend and participate in morning meetings/stand-ups to facilitate team communication.
• Answer residents’ questions about care, treatment plans, and illness progression.
• Monitor and adjust residents’ statuses based on changing needs and conditions.
• Organize and prioritize daily work by assessing resident needs and discharges.
• Complete documentation as required and ensure accuracy.
• Perform utilization review activities to provide cost-effective care.
• Coordinate care with residents, providers, and third-party payers.
• Oversee admissions and discharge activities; coordinate referrals.
• Relate positively with residents, families, staff and colleagues.
• Ensure timely charting and paperwork; maintain accuracy.
• Work independently and exercise sound judgment with physicians and payers.
• Communicate to promote cooperation among residents, families, and staff.
• Demonstrate strong time management in a high-volume, changing environment.
• Utilize intermediate computer skills in daily tasks.
• Maintain professional and respectful interactions with residents and families.
