Job summary

Qualifications

• Bachelor’s Degree in Nursing or Social Work is required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid and related programs.
• CCM certification a plus.

Responsibilities

• Communicate regularly with residents and families about care plans and treatment.
• Attend morning meetings to facilitate team communications.
• Monitor and adjust residents’ statuses as needs change.
• Complete documentation and required paperwork in a timely manner.
• Coordinate admissions, discharges, and referrals.
• Oversee utilization review and cost-effective care.
• Maintain professional interactions with residents, families, and staff.
• Demonstrate sound judgment when interacting with physicians, payers, and care teams.
