Job summary

Centerstone is seeking a Case Manager to coordinate services and care for patients, providing coaching, strategies and coordination of health and wellness resources to help them live more fully. You will connect with clients across clinic, home, and community settings, fostering whole-person recovery and resilience.

Bachelor’s degree in psychology, social work, or related field is required; experience with EHR is preferred, and a valid driver’s license is required.

Qualifications

• Bachelor’s Degree in psychology, social work or related field required.
• Experience with Electronic Health Record preferred.
• Valid driver’s license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support
• Coordinate the behavioral and primary care needs and develop wellness strategies
• Enters written documentation into Electronic Medical Record in a timely manner
