Job summary

Centerstone, a mental health nonprofit, is seeking a Case Manager to coordinate services for patients and provide coaching and strategies to connect physical and mental health. The role offers work across clinic, home, and community settings.

You will support clients in navigating care, developing treatment plans, and accessing resources, with emphasis on holistic recovery and ethics. Time type is full-time with competitive benefits.

Qualifications

• Bachelor’s degree in psychology, social work or related field.
• Experience with Electronic Health Records preferred.
• Valid driver’s license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refers, advocates, and links client and family for additional services as identified.
• Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills.
• Provides supportive and educational case management services to develop daily living and coping skills.
• Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.
• Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
• Connect with clients face to face in the office and community, telehealth, and by phone to address needs.
• Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.
• Attend weekly staffing and team meetings and completes assessments.
• Assist clients with developing natural support systems.
• Completes all required agency trainings to maintain 100% compliance.
• Maintains departmental productivity expectations.
• Attend and participate in clinical supervision.
